New Medical Group Patients

Important information for new Medical Group patients, including consent, consultation, authorizing release forms and our cancellation policy.

We ask that you complete the following forms and bring them with you to your scheduled appointment along with your insurance card and photo ID.

We will collect any money due to at the time of service so please be prepared to pay your copay, co-insurance, deductible or any balance due

No Show/Cancellation Policy

We understand that appointments sometimes need to be changed, so we ask that you call at least 24 hours in advance if you cannot keep your scheduled appointment to avoid a “no show”. Effective February 2022, we implemented a no show/ same day cancellation fee of $25 fee, which must be paid prior to your next appointment.

For patients who are delayed and arrive late for appointment, every effort will be made to see them the same day. However, if you are more than 15 minutes late, you will need to be rescheduled.
